BLD Tournament Participants and Spectators:
Big League Dreams is committed to providing a safe environment for participants and their spectators. In an effort to continue to slow the spread of COVID-19, Big League Dreams will be performing COVID WELLNESS SCREENINGS on all Players, Coaches, Umpires and Staff at Big League Dreams Sports Parks. This form should be completed 2 days, but not more than 5 days, before the event date.

This survey is identical, or similar, to one being used by student-athletes at high schools and colleges, hotels, convention centers and at other facilities and events across the country in order to provide a layer of protection for your visit. The screening also alleviates safety concerns related to the spread of Covid-19.
This survey simply asks a series of questions related to the player or coaches wellness and asks for your insurance information as well. You will not receive a bill and you will not pay any out of pocket expenses for this screening. If you happen to be uninsured, the CARES Act, passed by Congress in March of 2020, has engaged partners to administer these surveys on behalf of the federal government – again at no cost to you.
